About The Position
Sage Group HealthCare Staffing is seeking a Registered Nurse Hospice Case Manager for a 13-week travel/contract position in Northampton, MA. This role involves providing and coordinating care for assigned hospice patients in their homes, developing and implementing nursing care plans, and offering clinical oversight to an interdisciplinary team. The position requires adherence to professional nursing standards and documentation within 24 hours of patient visits. Additionally, the role includes providing bereavement support to patient families.
Requirements
- Registered Nurse: 2 years - Required
- Recent/Current Hospice nursing experience in a home health setting: 1 year within the past 3 years - Required
- Home Care Home Base (HCHB) charting experience - Required
- Must have valid Driver’s License and provide proof of personal Auto Insurance
- Must have reliable vehicle for patient visits
- Must be Covid Vaccinated or provide medical or religious exemption
- ASN or an equivalent – Required
- RN License for State applying or Compact if accepted - Required
- BLS Certification from AHA or American Red Cross - Required
Nice To Haves
- BSN or an equivalent – Highly preferred
Responsibilities
- Provide and coordinate care for assigned hospice patients in comfort of their homes.
- Develop and implement nursing care plans including Start of Care and admissions as assigned.
- Case management and provide clinical oversight to interdisciplinary team.
- Provide nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
- Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
- Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
- Notify physicians of significant changes in status.
- Document and complete charting for all patient visits within 24 hours.
- Provide Bereavement Support to patient families.
