About The Position

Mt Carmel Hospice is seeking a Hospice On-Call Registered Nurse to provide skilled end-of-life nursing care to hospice patients throughout the Columbus, Ohio service area. The role provides dedicated on-call nursing coverage from 5:00 PM to 8:00 AM, seven days per week, on a 7-on/7-off schedule. The RN assesses patient and family needs, develops and implements individualized plans of care, manages symptoms, coordinates services with the interdisciplinary hospice team, responds to urgent concerns, performs visits as needed, and provides caregiver education and support. The nurse also maintains timely documentation while promoting comfort, dignity, and quality of life for patients facing life-limiting illnesses. The ideal candidate is a compassionate, patient-focused RN who works independently, manages urgent clinical situations effectively, and is committed to supporting patients and families throughout the end-of-life journey. Requirements

Active Ohio RN license.
Current CPR certification.
Strong assessment, communication, and autonomous decision-making skills.
Ability to travel throughout the greater Columbus market and participate in on-call rotations.

Nice To Haves

Hospice, home health, oncology, critical care, ER, or med-surg nursing experience.

Responsibilities

Perform comprehensive patient assessments and evaluate changes in condition.
Provide symptom management and end-of-life nursing care.
Respond to after-hours, weekend, and holiday patient needs.
Coordinate care with physicians, hospice leadership, facilities, and interdisciplinary team members.
Complete admissions, routine visits, crisis visits, General Inpatient (GIP) visits, and Continuous Care (CC) visits as assigned.
Educate patients and caregivers about disease progression, medications, and comfort measures.
Ensure accurate and timely EMR documentation and compliance with hospice regulations.
Participate in quality improvement initiatives, team meetings, and ongoing professional development.
