This role involves providing skilled nursing care to patients at the end of life (EOL), conducting visits in facilities and/or homes. The Hospice Nurse will also provide on-call services after hours and complete scheduled skilled visits. Responsibilities include responding to patient and family calls regarding concerns, symptoms, changes in condition, or emergencies. The nurse will conduct home/facility visits to assess patients with critical symptoms such as pain management, respiratory distress, or emotional crisis. This position assists the RN Case Manager (RNCM) in carrying out the plan of care and performing specialized procedures under their direction and physician's orders. The role also involves active participation in interdisciplinary group (IDG) meetings to ensure the patient plan of care is current and issues are addressed. Additionally, the nurse will assist in reinforcing patient and family education regarding end-of-life care, assume responsibility for personal and vocational growth, and keep the RNCM informed of changes. Participation in self-development through further education, workshops, conferences, and professional organizations is encouraged. The nurse will also assist with emergent visits, including continuous care (CC) and companion care as needed, and perform other duties as assigned.
