This afterhours position is on call from home, but it is not fully remote. You must be available to travel for in-person visits when patient needs arise—including urgent symptom management and end-of-life care. You’ll have a rotating schedule every two weeks; Week 1 Monday, Tuesday, Saturday; Week 2 Sunday, Wednesday, Thursday, Friday. Responding to patient and family needs as they arise. There are no assigned caseload or scheduled visits—care is driven by patient needs and can vary throughout the shift. In this role, you will provide phone triage and support, travel to patient homes when needed, and deliver care for symptom management, active decline, and at times, support patients and families at the end of life—including during and immediately after death. These are often the moments when your presence matters most.
