About The Position

Their services include Skilled Home Health Care, Palliative Care, Adult Hospice Care, and Pediatric Care, all delivered with a team approach addressing practical, social, emotional, and spiritual needs to enhance quality of life. The San Francisco team is diverse and multilingual, serving a significant Chinese-speaking patient population. They are seeking a Registered Nurse to serve as a Hospice Nurse Case Manager, responsible for skilled nursing assessment, planning, and care to maximize patient and family comfort and health.

Requirements

  • One year experience as a professional nurse in the past three years or Bachelor’s degree in Nursing from an accredited School of Nursing.
  • Current license as a registered professional nurse in the State of California.
  • Bi-lingual in Cantonese, Mandarin, or Taishanese required!
  • Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car.

Nice To Haves

  • Experience in acute care/ home health and/or hospice setting preferred but not required.
  • Public Health Nursing certification highly desirable.
  • HPNA certification preferred.

Responsibilities

  • Provide care in the home and work as a member of the interdisciplinary team comprised of nurses, social workers, chaplains, hospice aides and physicians.
  • Coordinate all patient/family services and prioritization of needs, including implementation of the individualized plan of care.
