Per Diem/Part Time Hospice MSW Social Worker

About The Position

AdvisaCare is a home health and hospice provider focused on personalized in-home care and compassionate support for patients and families. In this role, the Hospice MSW Social Worker guides patients and families through the hospice journey by providing advocacy, counseling, and psychosocial, emotional, spiritual, and practical support. The position works under physician direction and participates in interdisciplinary team conferences and bereavement support.

Requirements

Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education (CSWE).
Active LMSW or LLMSW license in Michigan.
Prior experience working with patients and families.
Valid driver's license, car insurance, and reliable transportation.
Eligible to work in the United States.

Responsibilities

Perform initial psychosocial, emotional, spiritual, and bereavement assessments and help develop and implement goal-directed care plans.
Provide care under physician direction and participate in IDT conferences, observing, assessing, and sharing information about psychosocial, emotional, spiritual, physical, and financial conditions affecting patients and families.
Document patient and family visits, telephone contacts, and referrals accurately and promptly in patient records.
Reassess patient and family needs and provide counseling as required.
Provide crisis intervention and individual or family counseling when indicated.
Advocate for patients and their family units.
Develop and maintain relationships with community agencies and services to promote cooperation and facilitate referrals.
Provide ongoing counseling to patients and families about death and dying as needed.
Participate in the Bereavement Team as assigned.
Adhere to hospice policies.
