About The Position

Signature Healthcare at Home, part of The Pennant Group, is seeking a compassionate Licensed Practical Nurse (LPN) to join their hospice team in Lincoln City, OR. This field-based role involves providing direct patient care and support to hospice patients in their homes or in facility settings, focusing on comfort, dignity, and quality of life. The LPN will work under the supervision of a Registered Nurse Case Manager and collaborate with an interdisciplinary team to execute care plans and support families through end-of-life care. The position requires travel throughout the Lincoln City and surrounding coastal communities.

Requirements

  • Active Licensed Practical Nurse (LPN) license (Oregon or compact eligibility if applicable)
  • Minimum 1 year of nursing experience (hospice, home health, or long-term care preferred)
  • Valid driver’s license and reliable transportation (field-based role)
  • Ability to work independently and manage a flexible schedule
  • Compassionate, patient-centered care approach
  • Strong communication and interpersonal skills
  • Ability to navigate sensitive end-of-life conversations

Responsibilities

  • Provide skilled nursing care to hospice patients in accordance with the care plan.
  • Monitor patient condition and report changes to the RN Case Manager or interdisciplinary team.
  • Assist with symptom management, including pain control and comfort measures.
  • Administer medications as directed (within LPN scope of practice and state regulations).
  • Collaborate with RN Case Managers, physicians, social workers, chaplains, and aides.
  • Participate in interdisciplinary team (IDT) meetings and care discussions.
  • Support implementation of individualized care plans.
  • Educate patients and families on hospice care, medications, and disease progression.
  • Provide emotional support during end-of-life care.
  • Reinforce care techniques to caregivers to ensure patient comfort.
  • Maintain accurate, timely, and compliant clinical documentation.
  • Follow all Medicare hospice regulations and Oregon state requirements.
  • Ensure proper communication and documentation of patient status changes.
  • Adhere to infection control and safety standards.
  • Observe and report any safety concerns in the home environment.
  • Support quality initiatives and continuous improvement efforts.
