About The Position

Requirements

Current, unencumbered LPN license in the state of Oregon
Minimum 1 year of nursing experience
Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal abilities
Ability to work independently in a home-based setting
Valid driver’s license, reliable transportation, and current auto insurance

Nice To Haves

Home health experience preferred

Responsibilities

Deliver skilled nursing care to patients in their homes under the supervision of an RN
Administer medications, treatments, and wound care as prescribed
Monitor patient conditions and report changes promptly to the RN and care team
Educate patients and caregivers on care plans, medications, and disease management
Accurately document all patient interactions in compliance with regulatory standards
Coordinate care with physicians, therapists, and other healthcare professionals
Support patients in achieving optimal health outcomes and independence
