About The Position

The Hospice Licensed Practical Nurse (LPN) provides direct, skilled nursing services in accordance with the Plan of Care under the direction of the RN Case Manager. The role supports patients in spending quality time with loved ones and participating in activities they enjoy, while building compassionate relationships with patients and families. The nurse educates caregivers and collaborates with the care team to provide comfort, peace, and dignity.
