About The Position

As a Hospice Licensed Practical Nurse (LPN), you will provide critical nursing care to terminally ill patients in the comfort of their homes. Your primary responsibilities will include assessing patient needs, administering medications, providing emotional support, and ensuring that patients receive high-quality, dignified end-of-life care. You will collaborate closely with other healthcare professionals and family members to create a supportive environment for patients during their final stages of life.

Requirements

  • Current unrestricted Washington State licensure as a Licensed Practical Nurse.
  • Valid driver’s license and reliable transportation.
  • Meet insurance eligibility requirements for driving.
  • Ability to independently drive within Tri-Cities Chaplaincy’s service areas.
  • Pass a pre-employment drug screen (includes cannabis testing in Washington State).
  • Certification in Cardio-Pulmonary Resuscitation (CPR)/Basic Life Safety (BLS).
  • 2 years of nursing experience.
  • Ability to report to the assigned work location(s) as scheduled and maintain consistent, reliable attendance.
  • Demonstrate support and adherence to the agency’s Mission, Vision, and Values.
  • Maintain reliable personal transportation.
  • Maintain a valid driver’s license and vehicle insurance per state requirements.
  • Satisfy state and agency health requirements by providing proof of immunization, declinations, or approved accommodations as applicable.
  • Pass criminal background and drug screenings.
  • Meet the physical and environmental requirements of the position, with or without reasonable accommodation.
  • Maintain current licensure(s) and certifications(s) required for the role.
  • Read, write, speak, and understand English fluently.
  • Be available to work as assigned or scheduled.

Nice To Haves

  • Previous experience in hospice or palliative care.
  • Bilingual in Spanish and English, with the ability to speak, read, write, and communicate effectively in both languages.

Responsibilities

  • Provide critical nursing care to terminally ill patients in their homes.
  • Assess patient needs.

  • Participate in education and outreach activities to promote agency programs, including hospice care, advanced planning, and bereavement services.
  • Maintain knowledge of and comply with agency policies, procedures, and practices.
  • Protect patient and agency information per HIPAA guidelines.
  • Complete agency and departmental orientation and required training programs.
  • Complete ongoing education, policy reviews, and training by designated deadlines.
