About The Position

We are seeking a clinician who has a true passion for Hospice and will help us in building a truly special program for our patients in the Fort Wayne community.

Requirements

  • Current, active RN license in the state of Indiana
  • Minimum of three years of clinical experience in Hospice Care.
  • Strong understanding of Medicare home health and hospice eligibility criteria and documentation requirements.
  • Demonstrated ability to lead interdisciplinary collaboration, facilitate difficult conversations, and provide staff education.
  • Excellent written and verbal communication skills, with comfort presenting to clinical and non clinical audiences.
  • Proficient with electronic health record systems
  • Knowledge of Home health, hospice, and palliative care clinical models, eligibility criteria, and regulatory requirements Medicare Conditions of Participation, state regulations.
  • Knowledge of End of life care principles, symptom management, and advanced care planning.
  • Knowledge of Interdisciplinary team structures and documentation standards in home based care.
  • Ability to facilitate sensitive goals of care conversations with patients, families, and providers using empathetic, non directive communication.
  • Ability to use electronic health record systems, scheduling tools, and Microsoft Office to manage documentation, referrals, and workflow.
  • Ability to identify inefficiencies and implement process improvements to streamline transitions between service lines.
  • Ability to make sound clinical and programmatic decisions under time constraints while maintaining regulatory and quality standards.
  • Ability to build collaborative relationships with Sales, Intake, Home Health Leadership, and the Palliative Nurse Practitioner to ensure coordinated care.
  • Ability to travel independently between patient homes and offices, adapting to changing schedules and priorities.

Responsibilities

  • Coordinate and manage transitions between Home Health and Hospice service lines, ensuring continuity of care and regulatory compliance.
  • Conduct hospice clinical assessments and facilitate admission coordination to ensure timely, appropriate patient onboarding.
  • Lead compassionate, person centered goals of care conversations with patients, families, physicians, and interdisciplinary teams.
  • Train new staff including clinicians and account executives on criteria for selecting the right service line for each patient whether it is Home Health, Hospice, and other service lines we provide.
  • Monitor and participate in Home Health Interdisciplinary meetings to identify patients who may benefit from Hospice services.
  • Collaborate with Sales, Intake, Home Health Leadership, and the Hospice Leadership team to align communication, referrals, and transition planning.
  • Implement and support process improvements to enhance communication, reduce delays, and improve efficiency across service lines.
  • Maintain documentation in the electronic health record and ensure adherence to Interim Healthcare policies, federal and state regulations, and quality standards.
