About The Position
Albany Hospice is looking for an Intake Coordinator to be the welcoming first point of contact for patients, families, referral partners, and healthcare providers. Join a team dedicated to delivering exceptional hospice care with dignity, respect, and heart.
Requirements
- High School Diploma or GED
- 2+ years of healthcare administrative experience preferred
- Knowledge of medical terminology and insurance processes
- Strong communication, organization, and computer skills
- Ability to multitask in a fast-paced environment
- Valid driver's license and current insurance
Responsibilities
- Manage daily patient referrals and admissions
- Verify physician orders, insurance, and authorizations
- Maintain accurate patient records and intake documentation
- Build strong relationships with referral sources and community partners
- Track referral activity and support service growth opportunities
- Ensure HIPAA compliance and regulatory standards are met
- Represent Albany Hospice with professionalism and compassion
