About The Position

The Overnight Hospice LPN provides professional nursing care as a member of the hospice team and contributes to the assessment, planning, and evaluation of nursing services. Under the direction and supervision of the Hospice RN Case Manager, the LPN is accountable for the quality of care provided and delivers nursing care to assigned patients in accordance with the established plan of care. The role includes patient and family support, symptom management, communication with the interdisciplinary team, and participation in regular IDT meetings.

Requirements

Active, valid Massachusetts LPN license.
Graduate of a Licensed Practical Nursing program accredited by the National League of Nursing (NLNAC).
Active American Health Association BLS certification.
At least 2 years of experience as a Licensed Practical Nurse in an acute care, rehabilitation, or home care setting.

Nice To Haves

At least 1 year of hospice experience.

Responsibilities

Perform nursing visits to patients as assigned by the Hospice RN Case Manager or Hospice Clinical Manager.
Provide hospice nursing care to patients and families using current principles of palliative care and symptom management to support hospice outcomes as directed by the RN.
Observe, record, and report significant changes in a patient's healthcare status to the physician and Hospice RN Case Manager.
Participate in health teaching required by the patient and family.
Respect and support patients' and families' cultural beliefs and values, individual needs, health goals, and treatment preferences.
Communicate effectively with patients, caregivers, and members of the interdisciplinary healthcare team using interpersonal and therapeutic communication skills.
Attend regular IDT meetings.
Follow agency confidentiality principles and federal HIPAA guidelines.
