Requirements

  • Minimum 1 years of hospice experience, ideally in quality, compliance, or clinical documentation roles.
  • Demonstrated advanced knowledge and hands-on experience with Hospice Outcomes and Patient Evaluation (HOPE) submissions.
  • Advanced expertise in Microsoft Office Suite (Excel, Word, Outlook, and PowerPoint), including extensive experience with Excel data extraction, manipulation, analysis, and reporting.
  • Excellent communication, training, and interpersonal skills with the ability to communicate effectively with hospice staff in a positive and professional manner.
  • Strong attention to detail with a high degree of accuracy in documentation and data review.
  • Ability and willingness to travel to hospice sites as needed.
  • Valid state driver’s license and reliable automobile.
  • Proof of current automobile insurance.
  • Current health certificate/physical examination and TB testing.
  • Criminal background check.

  • Prior experience with hospice EMR system Wellsky (formerly Kinnser)
  • 2-years of Hospice experience

Responsibilities

  • Coordinate the implementation, accuracy, and compliance of the HOPE tool across all hospice teams and locations.
  • Maintain a HOPE submission timeliness of >95% for both licenses.
  • Ensure timely completion, documentation, and scoring of HOPE assessments per CMS guidelines.
  • Coordinate with the team to ensure symptom follow-up visits (SFVs) and HOPE update visits (HUVs) are completed within required time periods.
  • Serve as a subject matter expert and trainer for interdisciplinary teams on HOPE-related processes.
  • Audit and monitor HOPE tool entries for accuracy, completeness, and alignment with the patient’s clinical documentation including visit frequencies and plan of care orders.
  • When trends with specific clinicians are identified, working with their supervisor to provide additional education or performance improvement plans.
  • Submit and track HOPE data, ensuring timely, compliant, and error-free submissions.
  • Support quality initiatives by identifying trends in documentation, assessments, and compliance metrics.
  • Support the QA department with tasks such as chart audits, Plan of Care Correction/Order generation, clinical incident review, and documentation compliance monitoring.
  • Assist with additional quality and compliance initiatives as needed, including accreditation readiness, internal audits, and education rollouts.
  • Prepare and distribute regular reports and dashboards related to HOPE tool data, and quality indicators.
  • Maintain up-to-date knowledge of CMS regulations and internal policies affecting hospice quality and compliance.
  • Travel to hospice office locations for training, monitoring, and on-site support as needed (may include overnight travel).
