About The Position

The Hospice Field Nurse RN Case Manager provides individualized, quality hospice care to patients. The role includes conducting an initial nursing visit within three days of the admission assessment, building rapport and trust with the patient, and using assessment data to identify and communicate patient problems and priorities. The nurse collaborates with the interdisciplinary team to establish measurable patient goals and communicate the initial and ongoing plan of care. The position is based in the Marietta/Smyrna area of Georgia and is described as offering work/life balance and supportive management.

Requirements

• Possess a current Georgia Registered Nurse (RN) license and thorough knowledge of nursing principles.
• Demonstrate the ability to provide quality patient care.
• Be able to perform the essential functions of the position with or without reasonable accommodation.
• Demonstrate commitment to quality, timely, and cost-effective patient care.
• Possess a current Georgia driver’s license and a clean driving record.
• Have reliable transportation.
• Comply with the organization’s and position’s health screening and vaccination requirements, including annual TB screening and influenza vaccination.

Responsibilities

• Provide the initial nursing visit within three days of the admission assessment.
• Engage with patients promptly to develop rapport and a sense of trust.
• Identify patient problems, priorities, and issues based on assessment data.
• Communicate and collaborate on patient problems, priorities, and issues.
• Establish measurable patient goals.
• Communicate the initial and ongoing plan of care appropriately to the interdisciplinary team.
