About The Position

The Registered Nurse (RN) provides care and services to a group of hospice patients, visiting them where they reside, including private homes, skilled nursing facilities, and assisted living facilities. As a contributing member of the interdisciplinary team, the RN provides expertise in psychosocial assessments, interventions, and knowledge of community resources. The RN works in a Case Management, Admissions, or Resource role.

Requirements

Current Registered Nurse (RN) licensure in good standing in the state(s) of practice.
Ability to work well within a team environment and collaborate daily with interdisciplinary colleagues.
Ability to work closely with patients on a one-to-one level while working with support from a patient care team.
Ability to work independently on a day-to-day basis, including phone, email, and written communication and meetings with healthcare professionals.
Understanding of hospice rules and regulations.
Time-management skills to manage visits, documentation, meetings, and urgent needs.
Ability to learn an electronic medical records system.
Willingness to drive and travel across the service area, with reliable transportation, a valid driver’s license, and auto insurance.

Responsibilities

Provide care and services for a group of patients.
Visit patients in their residences, which may include private homes, skilled nursing facilities, and assisted living facilities.
Contribute to the interdisciplinary team by providing expertise in psychosocial assessments, interventions, and community resources.
Work in a Case Management, Admissions, or Resource role.
Collaborate with nurses, social workers, chaplains, volunteers, and CNA leadership.
Manage visits, documentation, meetings, and responses to urgent needs by prioritizing and triaging multiple concerns.
