As the Home and Community Bases Services Aide, you will provide support, assistance with personal hygiene and household functions for an individual to be able to remain in their own home. Requirements

  • Current driver’s license, vehicle insurance, and reliable transportation or access to public transportation
  • CT: Home Health Aides must have completed a training program approved by the state of Connecticut of not less than seventy-five (75)hours.
  • KY: Must successfully complete an aide training and competency evaluation.
  • LA: Current CNA certification or successful completion of a training program. Current First Aide Certification.
  • NY: Successful completion of a basic training program in home health aide services or an equivalent exam approved by the Department with written evidence of completion.
  • TN: Successful completion of a total of seventy-five (75) hours of training which include sixteen (16) hours of clinical training prior to or during the first three (3) months of employment and who is qualified to provide basic services, including simple procedures as extension of therapy services, personal care regarding nutritional needs, ambulation and exercise, and household services essential to health care at home.
  • WI: Must be listed on the Home Health Aide registry.
  • OH: No Additional Requirements.
  • FL: Successful completion of at least forty (40) hours of training in areas required by Agency for Health Care Administration or successfully passing the Home Health Aide competency test (Form AHCA 3110-1007 created by AHCA).
  • KY: Current CPR Certification.
  • MO: Must have at least six (6) months paid work experience as an agency homemaker, nurse aide, maid or household worker, or at least one (1) year of experience in caring for children or for sick or aged individuals. Successful completion of formal training in the nursing arts, such as a nursing aide or home health aide, may be substituted for the qualifying experience. To qualify as an Advanced Personal Care Aide must be a competency evaluated home health aide having completed both written demonstration portions of the test required by the Missouri Department of Health and Senior Services and 42 CFR 484.36.; or have successfully worked for the provider for a minimum of three consecutive months while working at least fifteen hours per week as an in-home aide that has received personal care training.
  • NC: For in-home aides not listed on the nurse aide registry, must have verification of core competencies by a registered nurse.
  • NY: Successful completion of a basic training program in home health aide services or an equivalent exam approved by the Department with written evidence of completion.
  • OH: Must have successfully completed an approved Ohio Department of Health State Tested Nurse Aide (STNA) training program and competency, OR meet the qualifications to be Medicare Certified Home Health Aide, OR have at least one (1) year supervised employment experience as a Home Health Aide or Nurse Aide and has successfully completed a competency evaluation, OR has successfully completed a Council on Aging Learning Advantages (COALA) Home Health training program, a certified vocational or other sixty (60) hour training and competency evaluation program that meets the requirements listed in rule 173-39- 02.11(C)(3)(a)(v)(b).
  • PA: Successfully complete one of the following: a competency OR training program developed by the agency that meets the requirements identified in 28 Pa. Code § 611.55 OR A home health aide training program meeting the requirements of 42 CFR 484.36 OR A training program meeting the training standards imposed on the agency as a provider in a Medicaid Waiver or other publicly funded program providing PCS services.
  • TX: Home Health Aide-Must have a minimum of one-(1) year full-time experience in direct client care in hospital or nursing facility, OR one (1) year full- time experience within last five (5) years in direct client care in agency setting, OR documented completion of training and competency evaluation program with home health experience within the last twenty-four (24) months, OR documented completion of competency evaluation program with home health experience within the last twenty-four (24) months, OR nursing student with documentation from director of programs or the dean of a school of nursing that states that the individual is a nursing student who has demonstrated competency in basic nursing skills aligned with the school's curriculum, OR be on Texas Department of Human Services (DHS's) Nurse Aide Registry with no findings of client abuse or neglect or misappropriation of client property. Must NOT be listed on the Employee Misconduct Registry.
  • WI: Must have forty (40) hours training or six (6) months of equivalent experience.
  • VA: Successful completion of a competency evaluation and in-service training.

Nice To Haves

  • Current CPR certification or ability to complete within 90 days of hire

Responsibilities

  • Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders
  • Support household tasks, meal preparation, and accompany clients to appointments or errands as needed
  • Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system
  • Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs
  • Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
