About The Position

As a Hospice CNA/Home Health Aide, you will play a vital role in supporting patients and their families during some of life's most intimate and challenging moments. Every day, you will help create a nurturing, respectful, and comforting environment, ensuring patients feel valued, cared for, and supported.

Requirements

Must have a reliable vehicle
Must be able to read and write in English and follow written and verbal instructions in English
Demonstrates interest in the welfare of ill and elderly
Must pass Aide Competency Evaluation Skills
Minimum of one year of experience as a Certified Nursing Assistant

Nice To Haves

Experience as a CNA, HHA, or similar role is preferred, but a heartfelt dedication to caring for others is essential.
Hospice experience preferred
High School Diploma preferred

Responsibilities

Provide hands-on assistance with daily living activities such as bathing, grooming, dressing, toileting, and mobility.
Monitor and report changes in patient condition to the hospice nurse or care team, ensuring timely interventions.
Deliver emotional, spiritual, and compassionate support to patients and their families, fostering a sense of comfort and understanding.
Help families navigate the hospice process, offering guidance and reassurance during difficult decisions and transitions.
Maintain a safe, clean, and organized environment for patients, promoting comfort and dignity at all times.
Document care provided accurately and timely and communicate effectively with the interdisciplinary team.
Assist patients with their daily activities
Care for patient's personal needs
