About The Position

This role involves providing direct patient care to hospice patients in their homes, assisted living facilities, or skilled nursing facilities. The CNA will be responsible for a caseload of 5-7 patients per day, spending 45-60 minutes with each. Responsibilities include personal care, documentation, supply management, communication with RN case managers, and training caregivers. The role requires significant driving between patient visits.

Requirements

  • High school diploma or equivalent.
  • Completion of CNA course.
  • Must attend 12 hours of in-service training supervised by a Registered Nurse on a rolling 12-month basis.
  • Competency of the CNA is assessed in the home every 14 days (onsite evaluation once a year unless there is an area of concern).
  • Has not had a lapse of 24 consecutive months where the aide has not provided services for compensation.
  • Current certification with State Board of Nursing, or current registration with Department of Facilities Service in the State of North Carolina/ South Carolina.
  • No prior experience required.

Nice To Haves

  • Two years experience with hospice, home health or oncology preferred.

Responsibilities

  • Provide direct patient care such as bathing, peri-care, dressing, mouth care, assisting to toilet, incontinence care, repositioning in bed, and assisting with meals.
  • Tidy the immediate patient area and change bed linens.
  • Chart within the facility EMR if patients are in facilities.
  • Care for patients and complete documentation at the bedside on a company-provided cellphone.
  • Maintain an adequate level of supplies in the vehicle for patient care.
  • Work closely with RN case managers to communicate patient status changes and concerns.
  • Train caregivers to provide personal care to their loved ones within the scope of practice.
  • Coordinate visit times with patients or caregivers.
