About The Position

Trinity Hospice in Denver is looking for a PRN chaplain to join their team. The primary function of this role is to provide spiritual care in the home to terminally ill patients and their families. This includes assessing spiritual needs, providing counseling, and facilitating visits from clergy or pastoral counselors to support the patient's spiritual well-being. The chaplain will also participate as a core member of the Interdisciplinary Group.

Requirements

Master of Divinity, preferred.
Per state guidelines, if any, for licensure.
One (1) year experience as a Chaplain in a health care institution preferred.
Working knowledge of community spiritual and other resources.
Good interpersonal skills.
Reliable transportation.
Current auto liability insurance and driver's license if personal vehicle is used for work.

Responsibilities

Provide spiritual care in the home to terminally ill patients and their families.
Assess the patient's and family's spiritual needs and provide counseling to meet those needs.
Make reasonable efforts to facilitate visits by local clergy, pastoral counselors, or other individuals who can support the patient’s spiritual needs.
Participate as a core member of the Interdisciplinary Group.
Attend interdisciplinary team meetings and participate in the patient care planning process.
Participate in Quality Assessment Performance Improvement process as required.
Use effective interpersonal relations and communication skills; facilitate the use of these skills by other team members to achieve desirable outcomes.
Meet mandatory continuing education requirements of the agency.
Demonstrate commitment, professional growth and competency by maintaining a working knowledge of public and private eligibility standards and requirements.
Promote Agency philosophy and administrative policies to ensure quality of care.
Demonstrate understanding of coordination and continuity of care by collaborating with the IDG.
Accept clinical assignments that are consistent with education and competence to meet the needs of the patients/families.
