About The Position

Trinity Hospice in Tulsa is growing and looking for a PRN chaplain to join our team! The primary function is to provide spiritual care in the home to terminally ill patients and their families. This role involves assessing the spiritual needs of patients and families, providing counseling to meet those needs, and facilitating visits from local clergy or pastoral counselors. The Chaplain participates as a core member of the Interdisciplinary Group.

Requirements

  • Master of Divinity, preferred.
  • Per state guidelines, if any (licensure).
  • One (1) year experience as a Chaplain in a health care institution preferred.
  • Working knowledge of community spiritual and other resources.
  • Good interpersonal skills.
  • Reliable transportation.
  • Current auto liability insurance and drivers license if personal vehicle is used for work.
  • Assess hospice patient and support system to identify spiritual needs as evidenced by POC, documentation, clinical records, and IDG conference minutes.

Responsibilities

  • Provide spiritual care in the home to terminally ill patients and their families.
  • Provide an assessment of the patient’s and family’s spiritual needs and provide counseling to meet those needs.
  • Make reasonable efforts to facilitate visits by local clergy, pastoral counselors or other individuals who can support the patient’s spiritual needs.
  • Participate as a core member of the Interdisciplinary Group.
  • Attend interdisciplinary team meetings and participate in the patient care planning process.
  • Participate in Quality Assessment Performance Improvement process as required.
  • Use effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.
  • Meet mandatory continuing education requirements of the agency.
  • Demonstrate commitment, professional growth and competency by maintaining a working knowledge of public and private eligibility standards and requirements.
  • Promote Agency philosophy and administrative policies to ensure quality of care.
  • Demonstrate understanding of coordination and continuity of care by collaborating with the IDG.
  • Accept clinical assignments that are consistent with education and competence to meet the needs of the patients/families.
