We are seeking a compassionate and dedicated Hospice Chaplain (Per Diem) to join our hospice team. The Hospice Chaplain provides spiritual, emotional, and bereavement support to patients and their families, respecting each individual's beliefs, values, and cultural traditions. The Chaplain collaborates with the interdisciplinary hospice team to ensure holistic, patient-centered care throughout the end-of-life journey.

Requirements

  • Bachelor's degree in Theology, Divinity, Pastoral Care, Religious Studies, or a related field (Master of Divinity preferred)
  • Ordination, ecclesiastical endorsement, or equivalent recognized by the applicant's faith tradition, if applicable
  • Experience providing spiritual care in hospice, home health, hospital, or healthcare settings preferred
  • Knowledge of end-of-life care, grief, bereavement, and hospice philosophy
  • Excellent interpersonal and communication skills
  • Ability to work independently and as part of an interdisciplinary team

Responsibilities

  • Provide spiritual assessments for hospice patients and their families based on individual beliefs, values, and preferences
  • Develop and implement individualized spiritual care plans as part of the interdisciplinary plan of care
  • Offer spiritual counseling, prayer, rituals, and other faith-based or non-denominational support as requested
  • Provide emotional support and guidance to patients, families, and caregivers coping with serious illness and end-of-life issues
  • Assist patients and families in finding meaning, hope, comfort, and peace during the hospice journey
  • Participate in interdisciplinary team meetings and collaborate with nurses, social workers, physicians, and other hospice staff
  • Provide bereavement support and resources to families before and after a patient's passing
  • Document spiritual care visits and maintain accurate, timely patient records in accordance with hospice regulations
  • Support hospice staff through education, consultation, and compassionate presence when needed
  • Maintain patient confidentiality and comply with all organizational policies and regulatory requirements
