About The Position

AHN Healthcare@Home is hiring a Certified Nursing Assistant to provide hospice care to patients in their homes in the New Kensington, Pennsylvania area. The role delivers compassionate, patient-centered care in collaboration with an interdisciplinary team focused on comfort, dignity, and quality of life. The position is field-based and requires travel to patient homes within an assigned territory. The CNA works independently while receiving support from a dedicated field education team and organizational leadership.

Requirements

• Hold an active Certified Nursing Assistant (CNA) certificate in the state where the employee works.
• High school diploma or certificate.
• At least one year of healthcare experience.
• Valid driver’s license.
• Ability to spend approximately 20% of the workday driving to and from patient locations.

• Hospice experience is preferred.

Responsibilities

• Provide hands-on care to hospice patients, including assistance with activities of daily living.
• Observe and report changes in patients’ conditions to support comfort and quality of life.
• Reinforce the care plan and help patients and caregivers understand what to expect during the hospice journey.
• Offer emotional support to patients and caregivers.
• Respond promptly and empathetically to patient needs.
• Accurately document care provided in the EMR.
• Travel to patient homes within the assigned territory.
