About The Position

Residential Home Health and Hospice is hiring a Certified Nursing Assistant to provide hospice care to patients in their homes in the Lebanon, Pennsylvania area. The role delivers compassionate, patient-centered care focused on comfort, dignity, and quality of life, working with an interdisciplinary team while also working independently in the field. The position is listed as 40 hours per week.

Requirements

• Active Certified Nursing Assistant (CNA) certificate in the state where the employee works.
• High school diploma or certificate.
• At least one year of healthcare experience.
• Current driver’s license and ability to spend approximately 20% of the day driving to and from patient locations.
• Successful completion of a drug screen prior to employment.

Nice To Haves

• Hospice experience is preferred.

Responsibilities

• Provide hands-on care to hospice patients and assist with activities of daily living.
• Observe and report changes in patients’ conditions to support comfort and quality of life.
• Reinforce the care plan and help guide patients and caregivers through the hospice journey.
• Offer emotional support to patients and caregivers and help them understand what to expect.
• Respond promptly and empathetically to patient needs.
• Accurately document care provided in the EMR.
• Travel to patient homes within the assigned territory.
