About The Position

Suncrest At Brighton/Suncrest Hospice aims to change expectations for hospice care by providing exceptional care and service to patients. It increases staff-to-patient ratios to support more clinical visits and lower clinician caseloads. The organization is a Community Health Accreditation Partner (CHAP)-certified hospice.

The Certified Nursing Assistant (CNA) provides resident care to assigned residents in accordance with resident assessments and care plans, under the direction of the Director of CNA Services or designee. The CNA coordinates care with other licensed care staff and follows company nursing policies, procedures, and safety guidelines. The role involves close, one-to-one work with patients, collaboration with an interdisciplinary care team, and the ability to work independently day to day.

Requirements

• Hold Certified Nursing Assistant (CNA) certification in the state or states of practice.
• Have a strong commitment to improving quality of life while partnering with community organizations in patient care.
• Be able to work well within a team environment and collaborate daily with care team members.
• Be able to work independently on a day-to-day basis.
• Be willing to drive and have reliable transportation, a valid driver’s license, and auto insurance.

Responsibilities

• Provide care to assigned residents in accordance with resident assessments and care plans and as directed by nursing management.
