About The Position

Residential Home Health and Hospice is hiring a Certified Nursing Assistant to provide hospice care to patients in their homes in the Lebanon, Pennsylvania area. The role provides hands-on, patient-centered care focused on comfort, dignity, and quality of life, working with an interdisciplinary team and traveling to patient homes within an assigned territory. The CNA works independently while supported by a dedicated field education team and organizational leadership.

Requirements

• Active Certified Nursing Assistant (CNA) certificate in the state where the employee works.
• High school diploma or certificate.
• Minimum one year of healthcare experience.
• Current driver's license.
• Ability to spend approximately 20% of the workday driving to and from patient locations.

• Hospice experience preferred.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting in collaboration with an interdisciplinary team.
• Assist patients with activities of daily living and observe and report changes in their condition.
• Support and guide patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand what to expect during the hospice journey.
• Respond promptly and empathetically to patient needs and accurately document care provided in the EMR.
• Travel to patient homes within the assigned territory.
• Work independently while collaborating with the broader care team.
