About The Position

Moments Hospice is seeking a hospice Certified Nursing Assistant to provide hands-on personal care that helps patients remain comfortable, clean, and cared for in private residences, assisted living settings, and care facilities. The role also provides companionship and supports patients’ families. The CNA works with an RN Case Manager as part of an interdisciplinary care team, with structured training and clear expectations.

Requirements

• Active Certified Nursing Assistant (CNA) certification.
• Ability to work in an electronic medical record system.
• Strong written and verbal communication and active listening skills.
• Excellent customer service, problem-solving, and time-management skills.
• Compassion, patience, and ability to handle emotional situations with care.
• Strong understanding of professional boundaries.
• Ability to lift 50 lbs. and a good understanding of body mechanics.
• Valid driver’s license and current auto insurance.

Nice To Haves

• Healthcare caregiving experience in home care, assisted living or memory care, skilled nursing, a hospital, or hospice.
• Current CPR certification.

Responsibilities

• Provide personal care and assistance with activities of daily living, including bathing, dressing, grooming, toileting, and skin care.
• Assist with lifting, transferring, positioning, range-of-motion activities, exercise, and ambulation.
• Assist with dining and nutrition.
• Review and follow each patient’s plan of care under the direction of the RN Case Manager.
• Provide comfort and companionship to patients and support to their families.
• Maintain electronic medical records.
