About The Position

Residential Home Health and Hospice is a nationally recognized leader in home health and hospice, and a consistent Top Workplace. We are hiring a Certified Nursing Assistant for a Hospice position providing care to patients in their homes in the Springfield, IL area. This role involves delivering compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life. The CNA will provide hands-on care, assist with activities of daily living (ADLs), observe and report changes in condition, and support/guide patients and caregivers. Responsibilities also include responding promptly to patient needs, accurately documenting care in the EMR, and traveling to patient homes within an assigned territory.

Requirements

• Active Certified Nursing Assistant (CNA) certificate in the state you work
• High School Diploma or certificate
• Minimum one year healthcare experience
• A valid driver’s license
• Ability to spend approximately 20% of your workday driving to and from patient locations

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Provide hands-on care to patients, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support comfort and quality of life
• Support and guide patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand what to expect during the hospice journey
• Respond promptly to patient needs with empathy and attentiveness and accurately document care provided in the EMR
• Work independently, but never alone
• Travel to patient homes within assigned territory
