About The Position

We are hiring a Certified Nursing Assistant for a Hospice position providing care to patients in their homes in the Novi area. This role involves delivering compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life. You will provide hands-on care, assisting with activities of daily living (ADLs), observing and reporting changes in condition, and supporting patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand the hospice journey. Responsibilities include responding promptly to patient needs, documenting care in the EMR, and traveling to patient homes within an assigned territory.

Requirements

• Active Certified Nursing Assistant (CNA) certificate in the state you work
• High School Diploma or certificate
• Minimum one year healthcare experience
• Valid driver’s license
• Ability to spend approximately 20% of your workday driving to and from patient locations

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Provide hands-on care to patients, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support comfort and quality of life
• Support and guide patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand what to expect during the hospice journey
• Respond promptly to patient needs with empathy and attentiveness and accurately document care provided in the EMR
• Work independently, but never alone
• Travel to patient homes within assigned territory
