About The Position

Residential Home Health and Hospice is hiring a Certified Nursing Assistant to provide hospice care to patients in their homes in the Palatine, Illinois area. The role focuses on compassionate, patient-centered care that supports comfort, dignity, and quality of life. The CNA works independently while collaborating with an interdisciplinary team and supporting patients and caregivers throughout the hospice journey. The position is eligible for a $5,000 sign-on bonus.

Requirements

• Active Certified Nursing Assistant (CNA) certificate in the state where the employee works.
• High school diploma or certificate.
• At least one year of healthcare experience.
• Valid driver’s license.
• Ability to spend approximately 20% of the workday driving to and from patient locations.
• Successful completion of a pre-employment drug screen, including screening for medical and recreational marijuana.
• Legal authorization to work in the United States.

• Hospice experience is preferred.

Responsibilities

• Provide hands-on patient care and assist with activities of daily living (ADLs).
• Observe and report changes in patients’ conditions to support comfort and quality of life.
• Reinforce the care plan and support patients and caregivers, including helping them understand what to expect during the hospice journey.
• Respond promptly and empathetically to patient needs.
• Accurately document care provided in the EMR.
• Collaborate with an interdisciplinary team.
• Travel to patient homes within the assigned territory.
