About The Position

Residential Home Health and Hospice is hiring a Certified Nursing Assistant for a hospice position providing care to patients in their homes in the Oak Brook, Illinois area. The role delivers compassionate, patient-centered care in collaboration with an interdisciplinary team focused on comfort, dignity, and quality of life. The CNA works independently while receiving support from the team and travels to patient homes within an assigned territory.

Requirements

• Active Certified Nursing Assistant (CNA) certificate in the state of employment.
• High school diploma or certificate.
• At least one year of healthcare experience.
• Valid driver’s license.
• Ability to spend approximately 20% of the workday driving to and from patient locations.

• Hospice experience preferred.

Responsibilities

• Provide hands-on patient care and assist with activities of daily living.
• Observe and report changes in patients’ conditions to support comfort and quality of life.
• Reinforce the care plan and support patients and caregivers, including helping them understand what to expect during the hospice journey.
• Respond promptly and empathetically to patient needs.
• Accurately document care in the EMR.
• Travel to patient homes within the assigned territory.
