About The Position

Home Care / Hospice Coordinator Make a Meaningful Impact in Patient Care Transitions We are seeking a Home Care / Hospice Coordinator to serve as a key liaison between the acute care setting and post‑acute home care and hospice services. In this role, you will help ensure smooth, timely, and compassionate transitions for patients and families while building strong relationships with hospital staff and community providers. This is an excellent opportunity for a clinically experienced professional who enjoys collaboration, education, and patient advocacy—while also contributing to referral growth and service excellence. Requirements

  • Bachelor’s degree or equivalent experience
  • Minimum of 2 years of experience in acute care, care coordination, customer service, sales/marketing, or a related role
  • Excellent verbal and written communication skills
  • Valid driver’s license and reliable transportation
