As our Home Health and Hospice Liaison, you will be the vital link between Anova Health Care System and the healthcare community. You will build and nurture relationships with hospitals, medical facilities, and community agencies, ensuring that referral sources, families, and prospective patients are well-informed about our comprehensive hospice services. Your ability to connect with others through compassionate communication, coupled with your clinical and marketing expertise, will be key to your success in this role. As a Home Health and Hospice Liaison with Anova Health Care System, you will cover a dynamic and diverse territory within Lawrence & Mercer County, including New Castle, Ellwood City, and Mercer! Your role will involve regular travel throughout these areas, allowing you to connect with a variety of healthcare professionals and community leaders. Your efforts will help ensure that patients across Allegheny County receive the compassionate care they need.

Requirements

  • Bachelor’s degree in marketing, nursing, or a related healthcare field is preferred.
  • A minimum of two years in healthcare sales/marketing, with a strong preference for those with home health and hospice/palliative care experience.
  • Current registered nurse or licensed vocational nurse credentials are a plus but not required.
  • Proven ability to establish and maintain strong relationships with healthcare professionals and community agencies.
  • A valid driver’s license is required for regular travel within the territory.

Responsibilities

  • Engage with hospitals, personal care homes, skilled nursing facilities, and other healthcare providers within your assigned territory to generate new referrals and maintain existing partnerships.
  • Develop and implement strategic business plans to achieve referral and admission goals. Present Anova’s home health and hospice services to potential referral sources and educate them on the benefits of our care.
  • Work closely with the Vice President of Business Development and internal teams to discuss opportunities, resolve challenges, and enhance service delivery.
  • Conduct initial meetings with patients and their families to introduce hospice services and assist in the eligibility assessment process.
  • Maintain accurate records of activities, communications, and referrals within the company’s CRM system.
  • Provide guidance and support to new team members as a sales mentor, as designated.
