About The Position

As a hospice aide, you provide compassionate, hands-on care to help patients remain comfortable and maintain dignity throughout their end-of-life journey. You support patients in their homes, including private residences and assisted living or skilled nursing facilities, traveling within an assigned service area. You work under the supervision of a registered nurse as part of an interdisciplinary hospice team, delivering personalized care that honors each patient’s wishes.

Requirements

• Approved hospice aide, home health aide, or nurse aide training and competency.
• Ability to travel as required by the role.
• Meet the applicable state certification requirement: AL—CNA; CA—CHHA; CT—Homemaker-Home Health Aide; DC, DE, FL, GA, PA, TX, VA, WI—CNA, HHA, or HA; IL—HHA or NA; KS, MO—Home Health Aide; OH—State Tested Nurse Aide; NJ—Homemaker-Home Health Aide.

Nice To Haves

• One year of direct patient care experience preferred.

Responsibilities

• Assist patients with personal care, mobility, and activities of daily living.
• Support nutrition, including simple meal preparation and feeding.
• Provide light homemaking support related to the patient’s care.
• Help patients maintain independence and participate in meaningful activities.
• Observe and report changes in a patient’s condition to the registered nurse.
