As the Admissions Nurse, you will be a vital member of the clinical admission team, responsible for the admission process of referred patients within the Capstone Hospice service area. You will serve as the first clinical impression for patients and families facing terminal illness, providing comfort and assurance. This role requires a registered nurse who understands the value of hospice care in alleviating pain and suffering. Success in this role depends on a compassionate nature, knowledge of caring for the terminally ill, and excellent interpersonal skills. The ultimate goal is to help patients live their final days with dignity and comfort. This role has the expectation of completing admissions Monday - Friday between 10 am and 7 pm.

Requirements

  • Graduate from an accredited school of nursing
  • Currently licensed as a registered nurse to practice in the state of Georgia
  • Minimum of 2 years as a registered nurse
  • Active patient contact within the past 3 years, to include hospice.
  • Active CPR certificate
  • Must have current valid driver's license, auto liability insurance and your own reliable transportation.
  • Must be able to provide evidence of annual TB test and other state required tests or examinations.
  • Covid vaccinated
  • Ability to work independently
  • Make accurate judgements
  • Strong nursing care processes and medication guidelines/side-effects
  • Strong nursing assessment skills
  • Proficient in the use of electronic medical record and hand-held tablet
  • Proficient in English (understanding and articulation)
  • Excellent communication skills
  • Attention to detail
  • Organized, efficient with strong time -management

Responsibilities

  • Complete at least 1-2 admissions per day when needed
  • Collaborate with physicians and other professionals to form and initial plan of care
  • Complete documentation within 12-24 hours of admission visit
  • Provide support and comfort according to individual patient's needs
  • Work with caregivers and patients to assess goals of care
  • Maintain accurate records
