About The Position

The Hospice Admissions Coordinator in Roswell plays a critical role in facilitating the admission process for patients entering hospice care, ensuring a compassionate and efficient transition during a sensitive time. This position serves as the primary point of contact for patients, families, and healthcare providers, coordinating timely admissions while maintaining compliance with regulatory standards. The coordinator is responsible for gathering and verifying patient information, assessing eligibility, and collaborating with interdisciplinary teams to develop individualized care plans. By managing communication and documentation effectively, the role supports both clinical staff and families to provide seamless hospice services. Requirements

  • High school diploma or equivalent; Associate’s degree or higher in healthcare-related field preferred.
  • Minimum of 2 years experience in healthcare coordination, patient admissions, or related administrative roles.
  • Strong knowledge of hospice care principles, patient rights, and healthcare regulations.
  • Proficiency with electronic medical records (EMR) systems and standard office software.
  • Excellent communication and interpersonal skills with the ability to handle sensitive situations compassionately.
  • The Hospice Admissions Coordinator utilizes strong organizational and communication skills daily to manage complex admission workflows and ensure all patient information is accurate and complete.

Responsibilities
