Requirements
- One year of RN experience required
- Licensed Registered Nurse in State of Indiana
- Current driver's license
- Must have reliable transportation and insurance
Nice To Haves
- One year Hospice experience preferred
Responsibilities
- Report to Clinical Coordinator and/or Intake Nurse each shift to learn of assigned visits/tasks for that day.
- Common assignments may include: Hospice evals or assessments, Urgent or change of condition visits, General visit coverage for a different RNCM when on PTO or when staffing demand is present, Tuck-in visits, GIP visits, Other misc. PRN visits.
- Appropriate follow-up / coordination of care with the involved RNCM or other involved team members after completing a visit.
