About The Position

We are seeking a compassionate and experienced Hospice Admission Registered Nurse (RN) to conduct comprehensive patient assessments, develop individualized care plans, and coordinate hospice services for patients in their homes. The Admissions RN plays a critical role in ensuring a smooth transition into hospice care while providing education, support, and guidance to patients and families.

Requirements

Current and unrestricted California Registered Nurse (RN) License.
Minimum of 2 years of RN experience; hospice, home health, palliative care, or public health experience preferred.
Strong clinical assessment, care planning, and communication skills.
Current CPR/BLS Certification.
Valid driver’s license, reliable transportation, and current auto insurance.
Ability to travel to patient homes and perform physical job duties, including standing, bending, lifting, and transferring patients as needed.

Responsibilities

Complete comprehensive hospice admission assessments, including physical, psychosocial, and medical history evaluations.
Develop and initiate individualized hospice care plans based on patient needs and goals.
Provide skilled nursing care, symptom management, and patient education.
Collaborate with physicians, interdisciplinary teams, caregivers, and community resources to coordinate care.
Educate patients and families on disease progression, medications, comfort care, and self-care techniques.
Monitor and document patient status, interventions, and outcomes in a timely and accurate manner.
Communicate changes in condition and obtain physician orders as needed.
