Position Summary: As a Homeless Services Nurse Clinician at HealthPoint, you’ll provide relationship-based, patient-centered care in various settings, such as permanent supportive housing (PSH) residences, homeless shelters, or via telehealth. This position is outreach-focused and utilizes a trauma-informed, harm-reduction approach to promote health, increase access to care, and support patients in achieving their healthcare goals. Compensation is dependent on skills and experience.

Requirements

  • AAST or BSN degree in Nursing from an accredited college or university with relevant certifications and one (1) year of experience required or equivalent combination of education and experience.
  • To be considered for a Nurse Clinician III, you must have a BSN and a minimum of 5 years of professional nursing experience, or an equivalent combination of education and experience totaling fifteen (15) years.
  • Degree in Nursing from an accredited college or university.
  • Unencumbered Washington State Registered Nurse license required.
  • National Certification in Ambulatory Care or similar type certification desired.
  • Beginning level of Word, Excel and Outlook required.
  • Experience with electronic medical records required.
  • Proof of vaccination for COVID-19 is required, prior to start. HealthPoint does not accept the Johnson & Johnson COVID-19 vaccine as proof of vaccination. If you have received the Johnson & Johnson vaccine, we ask that you provide documentation demonstrating proof of an alternate COVID vaccine or vaccine series.
  • All new employees are also required to show proof of immunizations and/or immunity to MMR (measles, mumps, rubella), Varicella, annual Influenza and TB QuantiFERON Gold Titer.
  • Additionally, if you work in a HealthPoint clinic, Tdap (within last 10 years) is required.
  • Hepatitis B. is required for clinical employees with potential exposure to blood/blood products.
  • All immunizations are a condition of employment.
  • Upon hire, employees must provide proof of their immunizations and/or immune titer results prior to starting or no later than their fifth (5) business day of employment.

Nice To Haves

  • National Certification in Ambulatory Care or similar type certification desired.

Responsibilities

  • Conduct trauma-informed outreach to people, in-person, face-to-face.
  • Perform independent visits at PSH site, operating under standing orders.
  • Partner with providers to deliver high-quality, compassionate direct patient care.
  • Facilitate connection to care at primary and specialty health settings and coordinate patient care with clinic teams and social service organizations.
  • Collaborate with our integrated health team partners in social work, care coordination, mental health, dental, pharmacy, behavioral health, etc. to provide holistic support.
  • Promote substance use disorder (SUD) treatment in a client-centered manner.
  • Use EPIC EHR, follow protocols and guidelines, accurately document patient information.
  • Demonstrate active listening, empathy, and cultural humility in every interaction.
  • Critical thinking, problem solving, time management, organization and teamwork.
