We are seeking a dedicated Case manager with a background in Social Work, Human Services, Psychology, or Behavioral Health to support individuals and families experiencing homelessness. Requirements

  • Bachelor's degree in Social Work, Human Services, Psychology, Counseling, or related field required
  • Minimum of one (1) year of experience in case management, social services, homeless services, or behavioral health
  • Strong case management and client engagement skills
  • Knowledge of homeless services systems, housing resources, and community programs
  • Excellent communication, documentation, and organizational skills
  • Ability to work independently and as part of a multidisciplinary team
  • High level of empathy, professionalism, and culture competence

Nice To Haves

  • Experience working with at risk populations, crisis situations, or housing programs preferred
  • Experience with HMIS, databases, or electronic case management systems preferred

Responsibilities

  • Conduct intake assessments, client interviews, and coordinated entry evaluations
  • Provide case management services, including goal setting, care planning, and ongoing client support
  • Support clients in rapid rehousing, housing stabilization, and homelessness prevention programs
  • Deliver crisis intervention, advocacy, and landlord mediation to maintain housing stability
  • Connect clients to community resources, mental health services, employment programs, and supportive services
  • Maintain accurate and timely HMIS data entry and case documentation
  • Monitor client progress and ensure compliance with grant funded program requirements
  • Prepare reports, case notes, and outcome tracking for program evaluation
  • Collaborate with social service agencies, housing providers, and community partners
  • Participate in team meetings, trainings, and outreach events
