About The Position

The Case Manager - Domestic Violence Programs position involves providing comprehensive support to residents in a program setting. Key responsibilities include conducting intake interviews, engaging residents in treatment through individual and group counseling, developing housing referral resources, and assisting residents in securing permanent housing. The role also involves assessing vocational and educational needs, facilitating referrals to community-based resources, and liaising with public assistance and Medicaid administrators. The Case Manager will participate in training, supervision, and staff meetings, and must be able to function independently during facility coverage, including crisis situations. Regular assessment of client safety and well-being, appointment scheduling for medical exams, and developing client-centered goal contracts are also core functions. The position requires responding to hotline calls, facilitating psychoeducational groups, coordinating on-site services, identifying and meeting concrete needs, and performing other duties as assigned by the Social Work Supervisor.

Requirements

  • High school diploma
  • Two (2) years of relevant training and experience, including experience working with victims of domestic violence; or Bachelor's degree in health or human services.
  • Two (2) years of crisis counseling experience preferred in a residential shelter for victims of domestic violence.
  • Experience and demonstrated skills in group counseling.
  • Training and experience in assessing the vocational and educational needs and aptitudes of clients.
  • Thorough familiarity and success with engaging clients.
  • Experience assisting eligible clients through the process of obtaining Public Assistance, Medicaid and other benefits.
  • Knowledgeable and sensitive to domestic violence issues.
  • Demonstrated sensitivity and experience with and/or knowledge of domestic violence within and against disenfranchised communities including individuals whom identify as lesbian, gay, bisexual, transgender, queer, and/or HIV-affected, the elderly and individuals with mental health issues.
  • Demonstrated ability in delivering client services including: communication and listening skills, understanding of human interaction and personality development, the ability to write and record case documentation, and demonstrated skills in group counseling.
  • Employee must be computer-literate and must have knowledge of Microsoft Office Applications.
  • Must have ability to be flexible of schedule as needed.
  • Excellent written and verbal skills are required.

Responsibilities

  • Conduct intake interviews to assess and evaluate residents for admission.
  • Engage residents in initial and ongoing treatment processes through individual and group counseling.
  • Develop and maintain housing referral resources and assist residents in locating and securing permanent housing.
  • Assist residents in referral and placement in community-based resources, including aftercare services.
  • Assess residents’ vocational and educational needs, aptitudes, and interests.
  • Develop and maintain placement and follow-up services after assessment and counseling.
  • Liaise with H.R.A. Public Assistance and Medicaid Administrators and Workers to assist eligible residents in securing Public Assistance and Medicaid.
  • Participate in training seminars to enhance and develop clinical skills.
  • Attend weekly individual case supervision, group supervision, monthly case conferences, and staff meetings.
  • Function independently in time of facility coverage, including responsibility for appropriate action in crisis situations.
  • Regularly assess the safety and well-being of clients.
  • Make appointments for residents’ physical exams.
  • Develop with the resident, a contract of mutually agreed goals and objectives.
  • Respond to Hotline calls as appropriate.
  • Facilitate psychoeducational groups.
  • Coordinate on-site services such as for child care, child therapy and recreation.
  • Identify concrete needs and provide on an as needed basis.
  • Perform other duties as assigned by the Social Work Supervisor.
