About The Position

This role supports and promotes a service environment for individuals in need of behavioral health and/or co-occurring services that is recovery-focused, welcoming, and attentive to the needs of individuals who may have experienced trauma. The Case Manager II will establish and maintain professional working relationships, monitor eligibility status for various benefits, and maintain up-to-date client information. Responsibilities include establishing rapport with clients, assessing needs, organizing and delivering services, and coordinating client needs with available resources to prevent hospitalization. The position involves providing intensive services to assist clients in maintaining outpatient recovery, assisting with transitions to lower levels of care, and participating in outreach efforts. Additionally, the role assists clients in accessing community resources for independent functioning, developing independent and community living skills, and creating individualized recovery plans. A key aspect of this role is maintaining strict confidentiality and managing a caseload of survivors experiencing homelessness and/or fleeing violence, creating goal plans, providing support and advocacy, and assisting with housing applications and related tasks. Mobile advocacy may be provided in community settings. Services are to be delivered in a trauma-informed, survivor-centered manner, following best practices such as housing first, progressive engagement, and harm reduction. The position requires a minimum of 85 billable service hours per month and utilizes collaborative/therapeutic documentation completed with the consumer on the same day of service. Seeking supervisory assistance for training and skill enhancement is expected. The role involves interacting with other professionals, discussing therapy or treatment, and acting as a liaison between inpatient psychiatric facilities for discharge planning and follow-up care. Documentation must be completed in accordance with agency policy, by the end of the next business day. Evening or weekend hours may be required, as may providing services in a community setting. Client transportation using a personal vehicle or agency vehicle is necessary.

Requirements

  • Must meet ODMHSAS and HOPE’s requirements.
  • Required to obtain and maintain DMHSAS CM II Certification.
  • Required to complete Therapeutic Options, or approved safety training and annual updates.
  • May be required to obtain and maintain First Aid and CPR Certifications.
  • In order to provide client transportation in private vehicle, must possess current, valid Oklahoma license, meet HOPE insurance requirements.
  • Ability to read and comprehend simple instructions, short correspondence, and memos.
  • Ability to write simple correspondence and clinical writing/documentation in medical records chart.
  • Ability to effectively present information in one-on-one and small group situations to families, clients, and other employees of the organization.
  • Ability to apply commonsense understanding to carry out instructions furnished in written, oral, or diagram form.
  • Ability to deal with problems involving several concrete variables in standardized situations.
  • Ability to seek out supervisory assistance for additional training when needed and make application of techniques or other skills to meet essential job duties and responsibilities.
  • Ability to relate to persons with severe mental illness and/or substance use in a positive, helpful and constructive manner.
  • Ability to establish and maintain effective and tactful working relationships with co-workers; to express ideas and facts clearly and concisely in written or oral form.

Responsibilities

  • Adheres to agency policy and procedures, CARF and DMHSAS standards and criteria, federal and state confidentiality rules and regulations, and any special funding source requirements including Medicaid and HCA regulations.
  • Supports and promotes a service environment that is recovery-focused, welcoming, and attentive to the needs of individuals who may have experienced trauma.
  • Attends annual core competencies training in relationship to individuals with co-occurring disorders or who have experienced trauma.
  • Establishes and maintains professional working relationships with supervisor and peers, and effectively works within a team or group setting cohesively.
  • Monitors eligibility status and makes application for Social Security, Title XIX (Medicaid), QMB, and other benefits the client may qualify for.
  • Maintains up-to-date address, phone number, contact persons, and financial information on all clients.
  • Establishes rapport with clients, assessing client needs; organizes, plans, and delivers face-to-face client services; and conducts follow up on the outcome of services provided.
  • Coordinates the needs of clients and family members with resources available and provides client advocacy as needed with an emphasis on preventing hospitalization.
  • Provides intensive services for clients to assist in maintaining outpatient recovery and utilizes the individual assessment to determine the frequency of services.
  • Assists clients in transitioning from a higher intensity level of services to lower levels of intensity services/appropriate other programs.
  • Participates in outreach and engagement efforts for clients and other professionals at inpatient levels of care to facilitate referrals and client participation in services.
  • Assists clients in finding and accessing community resources needed to maintain independent functioning.
  • Assists clients, individually and in groups, with development of independent and community living skills, social skills, interests, and leisure time activities which will encourage community involvement and integration.
  • Assists clients in developing an individualized recovery plan that focuses on personal and interactive problems, independent living skill needs, defining goals, and planning action reflecting interests, abilities, and needs.
  • Responsible for maintaining strictest confidentiality guidelines for interactions regarding consumer and family information.
  • Maintain a caseload of survivors, including single individuals and families, experiencing homelessness and/or fleeing from violence.
  • Create a goal plan with each household to address the steps needed to access housing.
  • Meet with survivors as often as necessary to evaluate progress in meeting steps in action plan, celebrate successes, provide comprehensive one on one support and advocacy to overcome challenges, and reassess and modifying goals as necessary to ensure success.
  • Provide direct services to assist survivors in meeting their goals. I.e.: assist in completing housing applications, rapid re-housing documents, completing credit repair tasks, and addressing other barriers to accessing housing.
  • Provide mobile advocacy for clients to ensure all survivors have access to the available services, which may include meeting at partner agencies, library, coffee shop, or other safe and mutually agreed upon locations.
  • Provide services in a trauma informed, survivor centered manner. Follow best practices including housing first, progressive engagement, and harm reduction.
  • Provides a minimum of 85 hours billable services with consumers per month.
  • Utilizes the philosophy of Collaborative/Therapeutic Documentation to meet clinical record keeping documentation standards and ensure documentation is completed with the Consumer to the extent possible and all documentation is completed on the same day of the date of service.
  • Responsible for seeking out supervisory assistance for additional training and to enhance skill set as needed.
  • Interacts with other professionals to discuss therapy or treatment, new resources or techniques, and to share information.
  • Works as a liaison between inpatient psychiatric facilities to assist in discharge planning and follow-up care from inpatient treatment.
  • Reports and documents services in accordance with agency policy and procedures.
  • Adheres to HOPE policy and procedure of having documentation completed by the end of the next business day.
  • May be required to work evening or weekend hours.
