About The Position

The Center for Human Development (CHD) is seeking an Outreach Where you Live (OWLS) Unsheltered Outreach Caseworker to join their new Unsheltered Outreach program in the Waterbury & Litchfield county area. The role involves overseeing various cases daily, providing unsheltered individuals with the necessary tools to achieve their goals. This includes maintaining a schedule for street outreach, managing equipment and supplies, and coordinating with external support providers. The caseworker will operate within the program's model and protocols established by the Western CT Mental Health Network and the Coordinated Access Network, ensuring open communication, support, and accountability among collaborating agencies.

Requirements

High school diploma or GED required
Two years’ experience working with adults with severe and prolonged mental illness, addiction/recovery or unsheltered populations
Reliable, registered & insured vehicle and approved driving record check
Approved criminal background check (CORI)

Nice To Haves

Two years in a human service organization preferred
Individuals with lived experience encouraged to apply
Bachelor’s degree from an accredited educational institution preferred
Familiarity with OWL program funding

Responsibilities

Oversees a variety of cases through different venues on a daily basis in the Waterbury & Litchfield county area.
Provides unsheltered individuals seeking assistance with the necessary tools to accomplish their goals.
Maintains a schedule including but not limited to hours of actual street outreach, use of equipment and supplies.
Maintains a schedule of external support providers that may be able to further the individuals’ attempts to end their homelessness.
Works within the program’s model as well as protocols established by the Western CT Mental Health Network and the Coordinated Access Network.
Establishes open communication, support and accountability among collaborating agencies.
