This position provides comprehensive case management services to identified chronic homeless individuals. The specialist is responsible for all aspects of their assigned caseload, including regular client meetings in the field and office, and ensuring all clinical files and reports are complete and up to date. The role involves intensive work with clients as they progress through the program continuum, conducting comprehensive assessments to identify and assist clients in overcoming barriers to housing and securing entitlements. Additionally, the specialist will develop and work with clients on individualized housing plans, build a network of support services, prepare clients for housing interviews, arrange housing appointments, and participate in case conferences. A key aspect of the role is liaising with clinical staff in other BRC or outside programs regarding common clients. Fire Safety and First Aid/CPR certification, or the ability to acquire them, is required. Related duties are assigned as necessary.
