About The Position

Alvita Care is seeking experienced RNs for homecare positions in Westchester County. We have clients throughout the county requiring shifts ranging from 8 to 12 hours. This role offers opportunities to perform skilled nursing visits and can accommodate full-time work or be integrated around an existing schedule. The positions involve providing one-on-one care in a home environment.

Requirements

NYS RN licensure.
BLS certification.
Minimum of One (1) year demonstrated experience as an RN.
Ability to put patients at ease and explain physician's instructions.
Ability to multi-task with excellent organization skills.
Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds.
Knowledge of medical terminology.
Team player and ability to work independently.

Responsibilities

Assess the needs of clients to develop a plan of care.
Consult with Management on complex/unique cases.
Provide a plan of care with support from appropriate multidisciplinary members to ensure services follow the plan of care through assessment, planning, coordination, counseling, teaching, and supervision to meet client/patient needs.
Monitor/Assess patients/clients' condition and the effects of the care plan.
Oversee/supervise the care provided to the patient by Licensed Practical Nurses and Paraprofessionals.
Manage, plan, coordinate, and execute the patient's safe discharge from service.
Ensure that physician's orders, discharge summary, and communication to team members, patient, and family members adhere to company and regulatory requirements.
Communicate and consult with the patient's MD when needed, and manage and coordinate all aspects of the assigned caseload.
Assist patients with activities of daily living.
Properly document the content and process of all services provided in the internal system.
Perform other related duties and participate in special projects as assigned.
