About The Position

The HomeCare Licensed Practical Nurse provides basic therapeutic nursing care according to established standards of practice under the direction and supervision of the RN. Provides care to those participants and caregivers whose care requirements are routine and standardized with predictable outcomes. Responsible for the development and implementation of homecare services for program participants. Relies on education, training and established guidelines to complete responsibilities. Initially requires instruction and guidance to accomplish job which should develop into working under general supervision.

Requirements

  • 1 year experience working with the frail elderly population.
  • 1 year experience working as LPN
  • Graduate of Accredited LPN program
  • Proficient in English, verbal and written communication skills and computer skills
  • Licensed as LPN in state of Louisiana or work permit and BLS certification

Responsibilities

  • Prescribed medications, therapies and treatments are administered in the home, if necessary, in accordance with practical nursing standards at all times.
  • Participant medical records, notes and physician orders are accurately reviewed and maintained; All participant care activities are accurately documented in accordance with documentation standards at all times.
  • Components of the plan of care within the LPN scope of practice and competency are implemented to promote desired outcomes at all times.
  • Assistance is provided with discharge planning and carrying out the participant's plan of care.
  • Performs the initial, 6-month, and annual assessment of the homecare needs of the frail elderly population.
  • Identifies and develops specific plans of care based on assessment.
  • Home visits required for each initial assessment, reassessment and for falls that require a home re-assessment. Also provides a home assessment if a new development causes changes in the home.
  • Assesses the safety of the home environment and whether the participant can be left home alone or must have supervision in the home.
  • The execution of participant's plan of care is effectively collaborated with the Interdisciplinary Team.
  • As an advocate for the participant, supports the participant's psychological, ethical, spiritual and cultural needs by collaborating with the Interdisciplinary Team.
  • Coordinates and authorizes all durable medical equipment necessary in the home.
  • Responsible for the maintenance and upkeep of each item.
  • Assists with diabetic shoes, grab bars appropriate seating in wheelchairs and all items related to improvement of their needs in ADLs.
  • Coordinates implementation of all home care services to include personal care services to ensure quality services are provided to meet participant needs.
  • Implements a Plan of Care addressing all the home care needs.
  • Actively participates in the Interdisciplinary Team meetings, presenting the prepared plan of care for the Participant/family/caregiver to sign showing their accountability.
  • Active participation on teams, work groups and/ or committees is provided as requested; anticipates needs and voluntarily assists others.
  • Outcomes of the team/committees' work are supported and put into practice.
  • Communicates participant changes to team members.
  • Able to implement crucial conversations with Participants, family members and caregivers regarding care in the home and safety situations
