About The Position

The Homecare LPN provides skilled nursing care in accordance with federal, state, and local laws, professional standards, and the Nurse Practice Act. Nursing services are provided under the direction of orders from a medical practitioner, such as an MD, DO, NP, or PA. The role includes caring for clients in home and community settings, including pediatric clients with specialized medical needs and adult clients. Available client schedules include day and night shifts in Saginaw, overnight and limited daytime shifts in Hemlock, Friday–Monday daytime shifts in Burt, and day and night shifts in the Chesaning/Saginaw area at SVSU.

First Day Homecare is a homecare provider specializing in pediatric Private Duty Nursing and other home- and community-based services. Its stated mission is to improve the lives of children and adults who are aging in place, medically complex, or living with chronic conditions through comprehensive care, oversight, and collaboration.

Requirements

• Graduate of an accredited school of professional nursing.
• Current license to practice as an LPN in the state of operation.
• Current CPR certification.
• Demonstrated written, verbal, and interpersonal communication skills.
• Licensed driver with an automobile insured in accordance with state and/or Agency requirements.

Responsibilities

• Provide professional nursing care as defined in the state Nurse Practice Act.
• Provide care in accordance with medical practitioner orders and the client’s care plan.
• Maintain client orders and document changes in orders in the medical record according to Agency procedures.
• Provide services within the clinical scope of practice, including personal care, monitoring and reporting findings, medication administration, client teaching, emergency intervention, and specialized pediatric services such as tracheostomy care, ventilator management, and tube feedings.
• Work collaboratively with other members of the client’s care team.
• Promote personal safety and a safe environment for clients and coworkers.
• Comply with Agency policies and procedures and applicable payor and accreditation standards.
• Participate in coordination of care and attend Agency meetings as requested.
• Perform other related duties and responsibilities as necessary.
