About The Position

The Home Health Aide (HHA) or Certified Nurse Assistant (CNA) will perform tasks assigned and supervised by an RN, focusing on patient care within the patient's home environment. The role involves assisting patients with daily living activities, maintaining a safe and healthy environment, and supporting patient well-being. The HHA/CNA will report any changes in the patient's condition to their supervisor and maintain accurate records of visits.

Requirements

  • High school diploma or general education degree (GED).
  • 75 hours of technical training.
  • One year of related experience and/or training, or equivalent combination of education and experience.
  • At least one year of experience working with the frail elderly.
  • Home Health Aide Certificate or Certified Nurse Assistant Certificate.
  • Completed a basic aide training program of at least 75 hours of training.
  • Ability to travel extensively in a geographic area.
  • Valid driver's license if traveling by automobile.
  • Ability to access clients' homes which will not be routinely wheelchair accessible.
  • Sufficient hearing, eyesight, and physical dexterity to perform a physical assessment of the client's condition and to perform and demonstrate client care.
  • Ability to give and receive information orally.
  • Must undergo a background screening through the Care Provider Background Screening Clearinghouse.

Responsibilities

  • Assists the patient with bathing, dressing, toileting and personal hygiene.
  • Assists in maintaining a healthful, safe environment.
  • Performs routine housekeeping tasks and chore services as related to a safe and comfortable environment for the patient (such as laundry, dishwashing, mopping, light dusting, sweeping), or as instructed by the professional nurse.
  • May assist with meal preparation, as assigned.
  • Assists the patient with transfers and ambulation.
  • Assists with simple procedures as ordered by the physician and approved and supervised by the nurse.
  • Assists the therapy personnel as needed with rehabilitative processes.
  • Promotes patient's mental alertness through involvement in activities and provides simple emotional and psychological support to the patient.
  • Reports any change in the patient's mental or physical condition or in home situation to immediate supervisor, or to the staff nurse.
  • Prepares a report of his/her visit on the day it is performed and incorporates same in the clinical record weekly or as directed.
  • Communicates the change in care plan and only performs assigned duties.
  • Understands definition of frequency (the number of occurrences of a repeating event).
  • Attends in-service as required by state and federal regulations.
