About The Position

LinkJobs is seeking a Home Visiting Nurse Practitioner to manage transitional and chronic care needs for homebound patients. The contractor visits patients in their homes, assesses their health, develops care-management and treatment recommendations, communicates findings to the office or supervising physician, and provides patient education. Visits may last from 30 minutes to 1.5 hours and may take place in homes with pets or household tobacco use.

Requirements

• Current Nurse Practitioner license in Georgia with no sanctions or restrictions.
• At least 1 year of clinical experience.
• Experience using Electronic Health Records (EHR).
• Strong communication skills.
• Comfort using a smartphone and email.
• Reliable transportation.
• Ability to work independently.
• Active individual malpractice insurance coverage.
• Willingness to spend 30 minutes to 1.5 hours with patients in their homes, which may include pets and/or household tobacco use.

Nice To Haves

• Clinical background in Adult, Family, or Geriatric specialties.
• Home care or home visiting experience.

Responsibilities

• Assess and evaluate patients’ current conditions.
• Obtain patients’ past medical histories and review their symptoms.
• Conduct general physical examinations.
• Review and reconcile medications.
• Screen patients for depression, alcohol use, and drug abuse.
• Identify diagnoses to support care management and treatment planning.
• Recommend appropriate measures, including medication prescriptions and skilled care needs.
• Communicate assessment findings efficiently to the office or supervising physician.
• Educate patients about disease processes, prevention, medications, and compliance.
