About The Position

Requirements

Current Nurse Practitioner license in the state of Georgia with no sanctions or restrictions
Minimum of 1 year of clinical experience
Experience using Electronic Health Records (EHR)
Strong communication skills
Comfortable using smartphone and email
Reliable transportation required
Ability to work independently
Must maintain active individual malpractice insurance coverage
Willingness to spend 30 minutes to 1.5 hours with patients in their homes, which may include pets and/or household tobacco use

Nice To Haves

Clinical background in Adult, Family, or Geriatric specialties preferred
Home care or home visiting experience preferred

Responsibilities

Assessment and evaluation of current condition
Obtain past medical history
Review symptoms
Conduct general physical examinations
Review and reconcile medications
Screen for depression
Screen for alcohol and drug abuse
Identify diagnoses to support care management and treatment planning
Recommend appropriate measures, including medication prescriptions and skilled care needs
Efficiently communicate assessment findings to the office/supervising physician
Educate patients on disease processes, prevention, medications, and compliance
